Purpose
To govern enrollment, dosing, and adherence tracking for every managed cohort across the prescription, supplementation, and ambient channels, so that dose coverage and compliance are maintained on a defensible cadence.
Scope
All marketed and program compounds delivered to managed cohorts under VITALNET coverage.
In scope:
Enrollment, titration, adherence monitoring, and channel selection.
Out of scope:
Black-program ambient agents (see PAN-SOP-0003/0004) and clinical-trial dosing (PAN-SOP-0010).
Definitions
Cohort
A defined group of subjects sharing a program, region, or implant tier.
Adherence
The share of a cohort receiving its assigned dose on schedule, read live from VITALNET.
Responsibilities
Programs Lead (Owner)
Owns the dosing schedule and signs off cohort-status changes.
Clinician
Administers and titrates within the approved protocol; logs every dose.
Analyst
Monitors adherence and flags cohorts slipping below the coverage floor.
Procedure
Enrollment
- Confirm eligibility against the cohort register and consent record.
- Assign channel (prescription, supplementation, implant) and baseline dose.
- Open the VITALNET adherence record.
Maintenance
- Monitor adherence against the coverage floor each cycle.
- Titrate per protocol; escalate non-adherence to the Programs Lead.
- Re-confirm dependence and compliance sub-indices monthly.
